Recovery Timeline for Anal Fistula Surgery

Main Causes:

The causes of the fistulas include infection in the anal gland, blockage of the fluids made inside the anus. Due to blockage, the bacteria form a swollen pocket of fluid and infected tissue, which becomes red. This is termed the abscess. If you leave the abscess untreated, then it will eventually grow and create a tunnel that runs through the opening of the gland. Hence, the main cause of the fistula is the abscess. Abscesses come from STI (sexually transmitted diseases), tuberculosis, and other illnesses like Crohn’s disease.

Symptoms of an Anal Fistula:

You may notice:

  • Pain in the anal area, especially when sitting or during bowel movements.
  • Swelling and tenderness around the anus.
  • Discharge of pus, blood, or fluid from the opening.
  • Itching or irritation around the skin.
  • Odor from the discharge.
  • Fever or feeling unwell (if there is an active infection).
  • Recurrent abscesses.

Types of Anal Fistula Surgery:

There are several surgical options. The right one depends on the type and complexity of your fistula.

Fistulotomy:

This is the most common surgery. The surgeon cuts the fistula open from the inside to the outside. This allows the tunnel to heal from the bottom up. It works best for simple, low fistulas.

Seton Placement:

A seton is a special thread. The surgeon places it through the fistula tract. The seton stays in place for several weeks. It slowly cuts through the fistula while allowing the surrounding tissue to heal. This helps protect the sphincter muscles.

Advancement Flap Procedure:

The surgeon removes the fistula tract. They take a piece of healthy tissue from the rectum and pull it down to cover the opening. This allows the area to heal without cutting the sphincter muscles.

Ligation of the Intersphincteric Fistula Tract (LIFT):

This is a newer technique. The surgeon makes a small incision between the sphincter muscles. They tie off the fistula tract and remove the infected tissue. This is effective for complex fistulas.

Plug Procedure:

The surgeon places a small plug made of collagen or other material into the fistula tract. The plug helps the tissue heal and close the tunnel. This is less common but can be an option for some patients.

Benefits of Anal Fistula Surgery:

  • Permanent closure of the fistula tract.
  • Relief from pain and discomfort.
  • No more discharge or odor.
  • Reduced risk of recurrent abscesses.
  • Improved quality of life.
  • Quick return to normal activities.
  • Minimal scarring.

How Is an Anal Fistula Diagnosed?

Your doctor will examine you to confirm the diagnosis.

  • Physical examination:The doctor will look at the area around your anus. They will check for openings, swelling, or discharge.
  • Digital rectal examination:The doctor inserts a gloved finger into your anus. They feel for the fistula tract and check the sphincter muscles.
  • Proctoscopy or anoscopy:A small, lighted tube is used to look inside the anal canal. This helps the doctor see where the fistula starts.
